Of the many conditions which result in a change in the shape or position of the toes, the curly toe stands out as a more unique deformity that is treated in a somewhat different manner than a standard toe deformity (like a hammertoe). This article will discuss this unique deformity, and ways to treat it effectively.
Due to a variety of reasons (but mostly because of one’s arch structure), the toes can gradually develop a change in their shape over the course of many years of standing and walking. The most common change is the development of the hammertoe, in which the first joint, or ‘knuckle’, of the toe pulls upward, while the middle of the toe flexes downward. Although rarely painful in and of itself, a hammertoe deformity can allow for excessive pressure from the top of a shallow shoe onto the toe. A corn will develop on the skin where the toe joint is prominent, which can eventually lead to pain. Another change that often develops is a rotation of the little toe towards the toe next to it. Often called a curly toe, this condition is the combination of a hammertoe deformity, and an actual inward rotation of the toe. The result of this abnormal toe position is usually a very painful corn that forms due to the pressure from the shoe on the toe, where the toe bone is now prominent due to the toe rotation. The difference between the corn that forms in this condition as opposed to that of a regular hammertoe is that this corn is less on the top of the toe than it is more along the side of the toe. The involved skin is still the ’top’ of the toe technically, but the corn appears on the side because the top of the toe has rotated to the side. In this case, not only does the top of the shoe irritate the corn, but the side does as well. A second pattern of corn can also form along side the outer edge of the nail where pressure from the ground builds up due to the toe’s rotation. This corn is often mistaken for an ingrown nail due to it’s closeness to the nail edge itself. Removal of the side of the nail in this case will only temporarily relieve the symptoms, as the toe pressure from the rotation will allow a corn to form again despite the nail edge’s removal.
The little toe is not the only toe that can curl inward. This can also be seen in the other smaller toes, although the fourth toe (the one next to the little toe) is the primary toe outside of the little toe to develop this. However, this toe does not tend to develop the same sort of painful corn as the little toe, due to the unique position of the little toe on the outside of the foot.
Treatment of this condition is fairly straight forward. One either conservatively lives with the condition or one has the toe straightened out surgically. Self-care with gentle filing of the corn with an emery board or pumice stone after bathing can flatten the corn and limit it’s potential to cause pain. Taping the toe out of a rotated position has limited benefit, and may actually cause additional discomfort. The use of a wider and deeper shoe seems to give the most benefit by reducing the external pressure on the toe. Padding the toe with gel, foam, or felt pads can provide some pressure reduction, although they need to be applied daily. Topical corn removers should be avoided as they contain skin acids that can potentially harm the surrounding good skin and create a chemical burn on the toe.
Surgery to repair this condition is possible, and is usually quite effective. This fairly simple procedure combines a fixing or straightening of the hammertoe deformity (by removing a small amount of bone near one of the toe joints), as well as a procedure to straighten out the toe rotation. Fixing the rotation can involve plastic surgery techniques to re-rotate the skin back around, or a technique to partially fuse the little toe with the base of the toe next to it to keep it stable and straight. The plastic surgery technique uses various skin incision orientations to effectively push the toe back into a non-rotated position. The fusion procedure removes part of the space in between the little toe and the fourth toe, and then stitches the skin together. Once this heals, the bases of the two toes will be as one. This has no effect on the way the foot functions in walking, and cosmetically is barely noticeable. The end result is a little toe that cannot rotate out of positionany longer. Recovery is fairly simple for both the bone and skin part of the procedure, with most people able to return to a normal shoe within three to four weeks. Complication rates are low, and the toe tends to stay in the corrected position for life.
The curly toe deformity is common, can cause discomfort in shoes, and yet is easily treated. Anyone suffering from this toe deformity should see their foot specialist (podiatrist) for evaluation and treatment options specific to their toe.

Aging Gracefully by Michael Logan
Is aging gracefully an oxymoron? Shouldn't we be raging against the night, to paraphrase an Irish poet?
I am 62, just started Social Security, and I am married to Julie, 15 years my junior, and I have an 11 year old son, Shane, and a 6 year old daughter, Hannah Marie.
My sister sent me an e-mail earlier this week including two video performances 50 years apart of a band called the Diamonds doing a song called "Little Darlin", I believe, and I felt a strong nostalgia for that time of my life, when my adventure was beginning, and that nostalgia has been a significant part of my emotional landscape in recent years, even as I handle the day to day aspects of being a Dad, and a husband, and a business partner to Julie.
There are dishes and laundry to do, and horn practice, and emotional meltdowns to handle, (some by the kids) and all the discussions that happen between husband and wife about holiday celebrations, which involve a certain level of stress, and then there are moments of appreciation for the beauty of our neighborhood and our safety and opportunity.
And then there are memories of parents and grandparents, and old friends from childhood scattered to the winds, and a longing to know what happened to them, and a thought about going upstairs to wake Julie before the kids get up, because who knows how long I (or she) will be around and I want her to know how much I love her. But instead, I am diligently building my new career.
And from my perspective, and supposedly, we older folks are better at seeing the big picture, I think about the incredible changes in the United States over the last 50 years, from the Kennedy assassination to Barack Obama.
Wait, did I just mention building a new career as an online entrepreneur?
I did, and that is perhaps the most important aspect of my aging gracefully.
Thirty years ago, I found myself in a situation where I had to chose to start over or die, and I did start over, and that is the most important lesson in aging gracefully for me.
I can start over even if I lose everything external to myself.
Part of that starting over involved some steps that needed to be repeated frequently, if not every day, so I learned to schedule regular practice, and as my new life unfolded, I undertook some other regularly scheduled practices which have impacted my ability particularly to be a Dad to small children, in other words, physical exercise, so that I can fling them around, or wrestle with the boy when he needs it.
So I have taken this experience of starting over and reiterated it several times, as a mid-life career changer, a late life father, and now as an online entrepreneur at 62. Some would say I have ADD, but I think I am flexible, and a book I read a couple of years ago, called The Brain That Changes Itself, backs me up.
Not too many years ago, no one knew of a concept called neurogenesis, which means that the brain grows new neurons, if I make the garden fertile, so to speak, so I see no reason not attend to the pillars of brain fitness which is what it takes to bring those replacement parts online.

This rise in obesity should be taken as seriously as any infectious disease epidemic. Overweight and physical inactivity account for more than 300,000 premature deaths each year in the U.S., second only to tobacco related deaths.
Clearly, many Americans are struggling to maintain a healthy body weight, which is putting them face to face with serious psychological, emotional, and physical health problems. Beyond the psychological and emotional hardships that overweight adults and children experience, is the physical toll that excessive weight takes on the body. Research studies indicate that a Body Mass Index (BMI) over 27 is directly correlated with increased incidence of disease and death. (See the next section to determine your BMI). Sadly, on average, people with obesity do not live as long as non-obese people. Obesity is the leading cause of preventable death in this country. You should be encouraged by the word "preventable" in the previous statement because even small reductions in weight can reduce your risk of early death from obesity.
Some of the medical problems associated with obesity that you should know about include heart disease, lung disease, diabetes, gall stones, cancer, and osteoarthritis.
Heart disease is one of the top risk factors associated with obesity. Positive Cognitive Aging by Michael Logan
Cognitive aging at the age of 62 can result in some scary feelings, a little worry, a little fear, because these days my vision or my memory is not quite what it was, and so on.
If you are my age, 62, then I am sure you and Bill Clinton feel my pain.
I grew up with the neuroscientific model that we had all the neurons we were going to have and all the brain circuitry that we were going to have by the time we were five years old or so, and that we would slowly lose those neuronal assets until we ended up in 'the home' staring vapidly at a fish tank.
That model is still so ingrained that at first blush I forget that we have made some wonderful discoveries about the human brain in the last few years, like the brain grows new neurons throughout the life span, so there are replacement parts available.
That is right, my cognitive aging does not need to describe a slow descent into Alzheimer's.
The process of growing new new neurons is called neurogenesis, and it can be encouraged.
Positive Cognitive Aging
But there is a catch, of course. Neurogenesis and its cousin, neuroplasticity, which describes the connections neurons make amongst themselves when they learn something new, can be encouraged if we take care of the what the research folks who are writing about this call the pillars of brain fitness.
Those pillars are physical exercise, good nutrition including anti-oxidants and omega 3 fatty acids, good sleep, (no problem with that one, right?), stress management, and novel learning experiences.
If you have not exercised for awhile, or do not like fish, which is the best source of omega 3 fatty acid, or are still living a high stress lifestyle, you may despair that neurogenesis is beyond what you can hope for.
Not true. You need to read this excellent e-book, called Brainfit for Life, written by Simon Evans,Ph.D., and Paul Burghardt,Ph.D. who are neuroscientists at the University of Michigan.
They write for the layperson, and their mission is to give us information we can use to age well.
In my book, that means increase neurogenesis and neuroplasticity.
Positive Cognitive Aging and Physical Exercise
Physical exercise is the most important pillar of brain fitness, but it does not mean that you have to buy an expensive club membership or hire a trainer.
It means you have build a way for there to be lots of deep breathing in your life, which could come from doing more of what you are already doing. If you are walking regularly, then walk for a little longer, a little faster, fast enough that it is hard to breathe and talk at the same time.
Of course once you start, you will want to start lifting weights and getting buff all over again.
One model worth looking at for its simplicity and economy is called the HIIT model, short for high intensity interval training, which you can do at home in ten minutes with an exercise ball or some dumbbells.
I can do that for positive cognitive aging.
Positive Cognitive Aging and Nurtrition
Broadly speaking, you get no more processed foods, which are full of appetite stimulants and high fructose corn syrup. You do get lots of fruit and vegetables because they are full of anti-oxidants which sop up free radicals that result from the incredible number of chemical reactions that happen in your noggin every second. Your brain uses 20% of the fuel you burn every day, but comprises only 2% of your weight, so lots of reactions happen, and free radicals are actually what cause aging.
Smoothies are a great way to get a daily dose of fruits, and juicing is a very good way to get those phytochemicals that our neurons need.
Your neurons are also about 70% omega 3 fatty acid, which we do not make very efficiently from food, unless you are eating lots of fish. Not sure about you, while I like eating fish, I am concerned about mercury so I like a supplement to make sure I am getting what Evans and Burghardt recommend. If omega 3 fatty acid is not replenished regularly, according to Evans and Burghardt, neurons get brittle and their conversations get garbled, leading to poor decisions.
Positive Cognitive Aging and Stress Management
Eustress versus distress. Neurogenesis is inhibited by the presence of unnecessary stress hormones, so stress management is a very important part of the cognitive aging process.
Stress management and mindfulness have important links, and biofeedback can be a key piece of stress management. The tool I like the best for stress management, because it combines mindfulness, cognitions, and breathing to learn how to manage the time between heart beats is called heart rate variability biofeedback.
The great thing about biofeedback is that it is learned so I practice it on the computer for 5-10 sessions, and then, more than likely, you will be able to cue a response simply by remembering your cue thought, or your breathing pattern, or both.
Heart rate variability biofeedback is a wonderful tool to use to help you drift off to sleep, and a full cycle of sleep stages is very important for your neurogenesis.
For example, your brain consolidates the days memories while you sleep, and also releases melatonin, which is a key daily event in keeping your brain healthy
Positive Cognitive Aging and Novel Learning Experiences
The last of the brain fitness pillars involves the novel learning experience, and it is the pillar which most enhances the connectivity of neurons or neuroplasticity, and that connectivity, or cognitive reserve is very important in providing re-routing around plaques in the brain.
The factor most important is the novelty factor, so if you are a counselor, you cannot read another counseling book and hope to enhance neuroplasticity.
The kinds of learning most endorsed for this pillar is the kind we experience when learning a new language or a new instrument.
