The human body is vast collection of complicated and amazing chemical reactions, and changing your metabolism, your weight, and your physical appearance takes time. Just as it takes a while to gain weight, it also takes a while, usually longer, to get skinny. Most people wouldn't be to intrigued to shell out twenty or thirty dollars for a book that promised to show you how to lose five pounds in a month.
However, by asking people that actually do have fairly decent bodies, they'll tell you how much work it requires. Especially if you are past your teenage years, when you could eat anything and not gain a single ounce. The trick then, is to accept the idea that a slow, gradual weight loss is the best way. And by far, without question, the best exercise for weight loss is simple walking. One foot in front of other, for twenty minutes a day or so. Of course, the more you walk the better, but if you are just starting out, twenty minutes a day is fine. Skip the fad diets, skip the hundred dollar a month gym membership, and go for a walk. You will be pleasantly surprised with the results.

What Constitues A Successful Balance Facility by Dr. A. R. Scopelliti
Clearly, the answer is one that is successful at treating patients with balance and vertigo type disorders. Our office has been sub-specialized in seeing patients with balance problems and fall risk, in particular vertigo and dizziness for some time now. It is well known that the simplest treatments for vertigo are the most useful, yet we continue to see patients inappropriately medicated with vestibular suppressant therapy, anti-anxiety drugs, and on and on.
The history of these patients typically includes that of a small fortune spent on diagnostic testing, which could have been avoided with appropriate clinical examination and management early on. The last statistic I read in the medical literature estimated that almost 99% of the MRI’s performed on patients referred with vertigo are normal. That’s a heck of a lot of money spent for a very small return, certainly not what I would consider an ideal use of the diagnostic dollar.
One of the main problems that I see everyday is that individuals are first seen by their primary care provider, which is now the way with insurance HMO’s. Being generalists, patients are managed such to rule out more threatening diagnostic entity’s, typically by imaging, (MRI). Since treatment for patients with vertigo and dizziness typically responds very quickly in our office, it would seem this course of action to be a much more sensible initial management plan in the multitude of cases where no life threatening problems are suspected. If no resolve is evident via this course, or, if on examination suspicion of more threatening diagnostic entities exists, then imaging can be performed. This plan only delays potential imaging by maybe one week, certainly not enough to have missed the boat, especially, in light of the statistic of 99% of these tests returning as normal.
The one most important factor related to all of this is the initial examination. The examiner should be so trained that it becomes readily apparent during the examination as to the urgency of specialized studies such as MRI, or not. In the latter, it should not be the next thing done. Further, all patients need to be evaluated for fall risk. Those with established fall risk should be managed with fall prevention and rehabilitative therapies congruent with their needs.
We see this as being overlooked more often than not. Our office currently is compliant with JCAHO goal and requirement re: falls-risk identification, assessment and prevention, effective 2005. Age 12 More Years Simply through Bad Habits by Dr. A. R. Scopelliti
Recent research on healthy aging shows that poor habits such as smoking, drinking too much alcohol, sedentary lifestyle and poor diet, can age you by 12 extra years. The findings are from a recent study that tracked nearly 5,000 British adults for 20 years, aged 18 and older and 44 years old on average, and they highlight yet another reason to adopt a healthier lifestyle. Overall, 314 people studied had all 4 unhealthy habits. Among them, 91 died during the study, or 29%. Among the 387 healthiest people with none of the 4 habits, only 32 died, or about 8%. “Bad habits” are said to include: smoking tobacco, drinking in excess of 3 alcoholic drinks daily for men, two for women, engaging in less than two hours of physical activity per week, and consuming fruits and vegetables less than 3 times daily. These cumulative habits substantially increased the risk of death and made people in this group seem 12 years older than people in the healthiest group. The most common causes of death in the study included heart disease and cancer, both believed to be related to unhealthy lifestyles. The study appears in the Archives of Internal Medicine. The healthy group included individuals who did not meet the criterion for “bad habits”.
It should be possible for most people to manage meeting the healthy test group’s habits, and further, each step in this direction reaps the reward of better health. The U.S. government generally recommends at least 4 cups of fruits or vegetables daily for adults, depending on age and activity level; and about two and 1/2 hours of exercise weekly, which equates to 30 minutes daily five days a week.
In a separate American study, no progress appears to be being made with respect to changing “unhealthy habits”. The rates at which Americans take part in exercise remains unchanged since 1997. We are a long way from where the health experts want us to be with respect to smoking. Overall, about 66% of adults in America are either overweight or obese. About one-third of people are in the obese category, meaning they have a body mass index of thirty or greater. In yet another AmericanUS study, extremely obese people, those who are 80 or more pounds overweight, live three to twelve fewer years than their normal-weight peers.

Just because an elderly person may be moaning, restless, grimacing or agitated does not definitely mean that they are feeling pain. One of the problems associated with diagnosing pain, according to the American Geriatric Society (AGS), is that there are no objective biological markers for pain. Therefore it may be difficult to diagnose a senior's pain.
However, under treating pain has been considered to be negligence, neglect, or even elder abuse.
So, how do you know if an elderly person you love is in pain? Although it may be tricky to obtain information, your most accurate source is the elderly person. And one way to find out is by asking the right questions.
For example, if you ask "Are you in pain?" your elderly relative could answer you with a simple "No." So you need to be more specific.
Ask questions such as "How have you been sleeping?"; "Do you feel achy?"'; "Does your knee still bother you?"; "Is your back sore?"
Health care professionals often use assessment tests to determine the intensity of pain. These tests compare a senior person's pain both before and after a particular therapy technique for pain - or specific medication - has been used.

Work is often stressful but sometimes the pressure at work can make you ill. When the workload and demands of an employer upon an employee are so excessive that they cause stress and the employer does nothing to remedy the situation the employee may be entitled to claim compensation.
Stress can be triggered by a number of factors. The risk of unemployment in a difficult economic climate is one factor. Often the risk of redundancy is worse and more stressful than the loss of a job. Another stress trigger during a recession can be the reduction in backup support and increased workloads.
Stress is not in itself an illness or psychiatric disorder. It has been described as - an excess of demands upon a person over and above their ability to cope '. It leads to anxiety and depression and may often bring about disabling conditions such as dyslexia and epilepsy.
Although an employer is entitled to expect an employee to cope with the ordinary pressures of the work he or she is paid to do, there is an obligation for the employer to take action when it is brought to his attention that this is not happening and the work has become unduly stressful. If an employer takes action to remedy the stressful conditions it is unlikely that he will be found at fault.
In the leading case of Hatton it was said that an employer who offers a confidential advice service, with referral to appropriate counselling or treatment services, is unlikely to be found in breach of duty towards the employee. Recent cases however have shown that an employer needs to do more than just offer counselling services when made aware that an employee is suffering from stress.
In the case of Intel Corporation (UK) Ltd v Daw, Mrs D had made her employers well aware that overwork was causing her stress. Counselling was made available but the Court of Appeal held that this was insufficient. It would not have resolved the problem. There was a duty for the employer to take action such as reducing and reorganising the workload once they were aware of the harm being suffered to Mrs D.
In another case the employer was told by the employee that she was - cracking up - under the pressure of the job. The six-month sabbatical she asked for was refused and instead offered counselling. When she suffered a complete breakdown the employer was held liable.
For an employer good employee communication is the answer to avoiding a claim. Regular appraisals should be conducted to identify any problems before they become an issue.
The employees most at risk from stress should be identified by a good employer. It is likely to be the most conscientious worker who is most affected. The perfectionist is more likely to be vulnerable to stress than a less meticulous worker.
Counselling services should certainly be offered and made available but as was said in Hatton this does not make such services a panacea by which employers can discharge their duty of care in all cases.
